In addition to the completed online application the following paperwork is required:
- A copy of your organization’s 501(c)(3) determination letter from the Internal Revenue Service including your Employer Identification Number (EIN)
- A copy of your organization’s audit report, compilation or review for the past fiscal year, completed by an independent accounting agency or CPA
The above paperwork must be forwarded to Ocean State Charities Trust after completing the on-line application. The online application deadline of February 1st would require the additional paperwork to be sent to Ocean State Charities Trust no later than March 1st. The online application deadline of August 1st would require the additional paperwork to be sent to Ocean State Charities Trust no later then September 1st.
